Home First is a national initiative to ensure that patients needing hospital care have access to the right support, to help them to recover well, avoid unnecessary re-admission, and maintain independence at home for longer.

The Home First VCSE Project is funded by Dorset CCG and Swip/C.  We are working in partnership with the Our Dorset Home First Programme to research how we can better coordinate, support and strengthen community groups, volunteers, and charities to play a pivotal role in supporting people over 65, with lower-level support needs in the community.

Community Action Network and Dorset Community Action are now calling for organisations who provide community support to help us to shape the design of the Home First Programme by:

  • Ensuring the VCSE has a strong influence in developing the theory of change.
  • Sharing and building best practise across communities
  • Creating stronger VCSE and statutory working relationships
  • Linking together and strengthening work already underway.

The Home First programme is all about helping people over 65 to recover in the comfort of their own homes wherever possible and we would like to start by hearing your views in these focus groups.

If you are a community group or organisation who currently supports people who have returned from hospital, or you help them to stay well at home, please register your interest by completing our State of Sector Survey here.

You will be invited to attend a series of virtual workshops we are holding between May until July to collaboratively design and develop this service.
