Home First is a national initiative to ensure that patients needing hospital care have access to the right support, to help them to recover well, avoid unnecessary re-admission, and maintain independence at home for longer.


Funded by Dorset CCG and Swip/C, we are working in partnership with the Our Dorset Home First Programme to research how we can better coordinate, support and strengthen community groups, volunteers, and charities to play a pivotal role in supporting people over 65, with lower-level support needs before, during and after hospital admissions.

Through our Focus Groups, Creative Workshops, and our Options Review we have co-designed a proposed model – a single point of access for statutory services to broker VCSE support not only for hospital pathways but also for adult social care too.

Phase Three

In the third phase, we will be testing the feasibility of this model by identifying the types and levels of VCSE support required with patients and carers who have accessed care through the A&E department in Dorset County Hospital.

Dorset Community Action and Community Action Network are now calling for organisations who provide community support to help us to develop and build the VCSE Home First Community Response offer by:

  • Sharing and building best practise across communities
  • Creating stronger VCSE and statutory working relationships
  • Linking together and strengthening projects that are already underway
  • Developing a knowledge bank of not for profit, local charity and community groups who can help.
